# What to have beside you at the visit

A few notes can keep the most important concern from getting lost. Leave blanks where you need the clinician’s help.

Bring what you know; mark what you want help finding.

Note to bring

Question it helps you ask

Your goal

Write down what made you seek care and what you hope will change.

The purpose

What condition are we treating, and what will show whether the care helps?

Your medicine list

Bring the names of prescribed medicines, other products and supplements you use.

The review

Which of these matters for the option we are discussing?

Past care

Note previous treatments, what felt useful and what you found hard.

The fit

How does this plan account for that experience?

## Health history deserves its own space

Semaglutide and tirzepatide label cautions cover thyroid cancer history, pregnancy plans, and past pancreas or gallbladder problems.

Serious stomach conditions and eye damage linked to diabetes also matter in those labels.

Those labels also flag other diabetes medicines and planned surgery as relevant.

If a family diagnosis is unclear, write it as a question rather than trying to guess the exact name.

## If blood tests are proposed

For each proposed test, ask what question it answers, where it takes place, who reviews the result and whether you pay separately. Ask how existing results can be shared.

As of September 26, 2026, federal rules do not generally require blood tests for every prescription; state requirements and a person’s clinical needs still count.

## Make room for the part that is hard to say

It may be the cost, a worry about an injection or a past visit that felt rushed. Put that concern near the top of your notes. You can read it out if that feels easier.

### The question I want answered first

What I do not want to leave the visit wondering:

Why the history questions matter These are issues named by the medicine labels, not a promise about what any service will ask.

Semaglutide (Wegovy) product label
